Attendant Care (ATC) service supports Members in remaining in their own homes with their families and/or participate in work or community activities while receiving assistance for mobility, self-care, socialization, communication, cognition, household activities, community participation, and employment. ATC supports Members to be as independent as possible in their own homes, communities and jobs. Families of loved ones and Members choose how and when their support is provided and determine their customized service to best support their unique needs. This service adequately provides for the Member’s basic needs and supports them to have optimal health and wellness.
Habilitation Care (HAH) is the process by which members are assisted in acquiring and maintaining life skills that enable them to cope more effectively with personal and environmental demands while raising the level of their physical, mental, and social efficiency. Our agency ensures that Members assessed for Habilitation outcomes, based on the responsible person’s priorities and goals, identified in the planning document to improve, or learn new skills.
Respite Care (RSP) service provides short-term care to relieve caregivers. Respite is an interval of rest and/or relief to a family member or other person caring for a member, which is aimed at improving the emotional and mental well-being of the member.
Day Treatment (Day Programs) provide supervision for members, community outings and activities with helping members to develop their skills in becoming more independent. This service is primarily offered in community settings. It is intended in helping members excel in learning activities of daily living, safety skills and socializing.
Developmental Homes are family homes in which our licensed caregivers provide full-time care services and supervision for up to three DDD individuals with developmental disabilities. Child developmental homes serve members under the age of 18 and adult developmental homes serve members ages 18 and over.
